The documentation required depends on the type of qualifying event. Examples of required documentation may include:

  • Marriage: Copy of marriage certificate

  • Divorce: Copy of divorce decree or legal separation agreement

  • Birth of a child: Copy of birth certificate or hospital record

  • Adoption of a child: Copy of adoption decree or placement agreement

  • Death of a dependent: Copy of death certificate

  • Loss of other coverage: Written confirmation of loss of coverage, including the end date, such as a letter from the prior carrier or employer

  • Gain of other coverage: Written confirmation of new coverage eligibility and effective date
