Supplemental Life Evidence of Insurability Waiver (EOI)
Evidence of Insurability, sometimes called EOI or proof of good health, is a medical questionnaire or review that is required before certain Supplemental Life and AD&D coverage amounts can be approved.
Newly hired or newly eligible employees may elect Supplemental Life coverage up to the guaranteed issue limit without completing Evidence of Insurability. Coverage amounts above the guaranteed issue limit require Evidence of Insurability and must be approved by the insurance carrier before they become effective.
Evidence of Insurability may also be required if you enroll late, meaning you did not elect coverage when you were first eligible, or if you request certain increases in coverage during Annual Enrollment. Spousal coverage also requires Evidence of Insurability depending on the amount requested and whether coverage was elected when first eligible.
Any coverage amount that requires Evidence of Insurability will not become effective unless and until it is approved by the insurance carrier. The EOI must be submitted to UnitedHealthcare within 30 days of electing coverage or the coverage will be terminated.
